Understanding Knee Joint Problems

Osteoarthritis

Knee osteoarthritis involves progressive breakdown of the articular cartilage that lines the joint surfaces. This cartilage provides a smooth, low-friction surface that allows bones to glide against each other easily. As cartilage wears away, bones may begin rubbing against each other, causing pain, inflammation, and stiffness.
 
The body attempts to compensate for cartilage loss, but these compensatory changes often create additional problems. Bone spurs may develop. The joint capsule may thicken. Muscles around the knee may weaken. Osteoarthritis tends to worsen over time.

Meniscus Injuries

The menisci are C-shaped pads of fibrocartilage that act as shock absorbers between the femur and tibia. Meniscal tears can occur from acute injury or develop gradually with age and wear. Symptoms may include pain, swelling, catching or locking sensations.

Ligament Damage

The knee’s ligaments—including the ACL, PCL, MCL, and LCL—provide stability and guide joint movement. Ligament injuries range from minor sprains to complete tears. Chronic ligament insufficiency can accelerate joint degeneration over time.

Tendon Problems

The tendons surrounding the knee can develop chronic irritation or degeneration, particularly with repetitive stress. Patellar tendinopathy and quadriceps tendinopathy cause persistent pain that limits activity and often resists conservative treatment.

Most patients with chronic knee pain have traveled a familiar path. The journey often begins with anti-inflammatory medications and activity modification. Physical therapy can help strengthen supporting muscles but cannot restore damaged cartilage.

Corticosteroid injections can provide temporary relief by reducing inflammation, but their effects often diminish over time. Research suggests repeated corticosteroid injections may actually accelerate cartilage breakdown. Viscosupplementation aims to improve joint lubrication but addresses symptoms rather than structural damage.

When conservative measures fail, knee replacement surgery is frequently presented as the primary remaining option. While knee replacement has helped millions, it involves major surgery with inherent risks and requires extended rehabilitation. Many patients want to explore alternatives before committing to joint replacement.
